Rhinoplasty Recovery Timeline: What Really Happens Week by Week
Recovery is the part of rhinoplasty that surprises patients the most. Surgery takes 2–3 hours; healing takes a year. The cast comes off at day 7. Most bruising fades by day 10. You'll be socially presentable by week 2, back to light cardio at week 3, full activity at week 6. The bridge looks beautiful by month 3; the tip continues to refine through month 12 (and through 18 in thicker-skinned patients). Sleep elevated, hydrate, walk daily, and trust the timeline.
Recovery is the part of rhinoplasty that surprises patients the most. The surgery is over in three hours; the healing takes a year. Here's an honest week-by-week walkthrough so you arrive prepared, not anxious.
Day 0 — surgery day
The operation lasts 2–3 hours under general anesthesia. You'll wake up with a small splint on the bridge of your nose, internal nasal splints (very small, soft tubes that allow breathing while the septum heals), and possibly a small dressing under the nostrils to catch oozing. Pain is usually mild to moderate and well managed with paracetamol or simple oral analgesics.
Days 1–3 — peak swelling
Swelling and bruising peak around day 2–3. Your face will look puffy, eyes may bruise (especially without piezo), and you'll breathe through your mouth because of the internal swelling. Sleep with two pillows. Use cold compresses (not ice directly) on the cheekbones for 20-minute intervals. Saline rinses every few hours.
Day 7 — cast off
Cast comes off at the one-week appointment. Your nose will look nothing like the final result — it will be swollen, possibly slightly upturned, tip stiff. This is normal. Most bruising has faded. Many international patients fly home around day 8.
Weeks 2–3 — back to social life
By the end of week two, most patients are presentable in public without makeup. Light desk work is fine. Avoid any activity that could bump the nose. No glasses on the bridge for six weeks (use tape on the forehead if needed, or contact lenses).
Weeks 4–6 — back to most activity
Light cardio at week 4. Heavy lifting and contact sports at week 6. The nose still feels “stuffy” at times because internal swelling hasn't fully resolved.
Months 3–6 — refining
Around 70–80% of swelling has resolved. The bridge is settling, the tip is starting to soften but is still firm. Friends will say you look great. You may start seeing the new shape in a way you didn't before.
Months 6–12 — final result
The last 20% of swelling — most of it in the tip — takes the longest to resolve. The nose continues to refine in subtle ways through month 12 and, in thicker-skinned patients, through month 18.
What helps recovery
- Sleep elevated for the first three weeks.
- Stay hydrated and avoid salt-heavy meals.
- No alcohol for at least two weeks.
- No smoking — at all.
- Walk daily; gentle movement helps drainage.
- Be patient with the tip.
