What is cheek enhancement?
Cheek enhancement restores midface volume, lifts the under-eye, and creates the elegant elevation that anchors a balanced, youthful face. At Dr. Havva Duru İpek's Nişantaşı clinic, every cheek plan is built around a thorough assessment of your midface structure and proportions, delivering natural, refreshed results — never overfilled.
Choosing the right approach depends on your anatomy, the degree of volume loss or desired enhancement, and whether you are seeking a temporary or permanent result.
What it does and doesn't do
Cheek filler (non-surgical, temporary). The most popular starting point for midface enhancement. Premium hyaluronic acid filler is placed with precision along the cheekbone and midface to restore volume, lift the skin, and create elegant, natural-looking elevation. Results are immediate, fully reversible, and typically last 18-24 months depending on the product used. For most patients, 2-4 syringes achieve a noticeable yet natural result. Anatomically targeted techniques deliver elevation, not width, avoiding the "pillow face" effect entirely.
Fat transfer to cheeks (surgical, permanent). An excellent permanent alternative to filler, using your own harvested fat to restore midface volume. Fat is gently extracted from a donor site (typically the abdomen or thighs), purified, and precisely reinjected into the cheeks. Because it uses your own biological tissue, the result feels entirely natural and carries no risk of foreign body reaction. A proportion of the transferred fat integrates permanently, making fat grafting more economical than repeat filler over the long term.
Malar implants / cheek implants (surgical, permanent). For patients seeking a permanent, structural elevation of the cheekbone projection itself, custom malar implants placed over the zygomatic bone deliver lasting, defined results. Incisions are made inside the mouth, completely hidden, and the implants are sized and shaped to your specific anatomy.
Who is it for?
Choosing the right approach depends on your anatomy, the degree of volume loss or desired enhancement, and whether you are seeking a temporary or permanent result. Malar implants are an ideal option for patients with naturally flat or underprojected cheekbones seeking permanent structural change. Fat transfer suits patients wanting a permanent result without any foreign material. Filler is the starting point for patients wanting immediate, reversible improvement.
Procedure process
At Dr. Havva Duru İpek's Nişantaşı clinic, every cheek enhancement plan begins with a thorough assessment of your midface structure and proportions. For filler, premium hyaluronic acid product is placed with precision along the cheekbone and midface. For fat transfer, fat is gently extracted from a donor site, purified, and precisely reinjected into the cheeks. For malar implants, incisions are made inside the mouth and custom implants are sized and shaped to the patient's anatomy before being placed over the zygomatic bone.
After the procedure
With filler, results are immediate and downtime is zero. We recommend sleeping on your back with your head slightly elevated for the first 2-3 nights after filler treatment, to prevent pressure from shifting the product while it integrates with surrounding tissue; normal sleeping position can be resumed after 72 hours. With fat transfer, the initial swelling is more significant than with filler, as would be expected of a surgical procedure using harvested tissue.
Risks
Anatomically targeted filler technique is used specifically to deliver elevation, not width, and to avoid the "pillow face" effect that can result from overfilling. Unwanted width only occurs when filler is placed on the sides of the cheek rather than at the top of the cheekbone — correct placement is what avoids this risk. Fat transfer carries a more significant initial swelling than filler, reflecting its surgical nature, though because it uses the patient's own tissue it carries no risk of foreign body reaction.
Related procedures
Cheek enhancement is often planned alongside chin refinement, jawline contouring and lip refinement as part of a full facial-balance plan.
Frequently asked questions
Does cheek filler hurt?
The procedure is very comfortable. Premium filler products contain lidocaine (a local anaesthetic), and topical numbing cream is applied beforehand. Most patients describe feeling mild pressure, not pain.
Will cheek enhancement make my face look wider?
Not when placed correctly. Expert filler placement targets the apex of the cheekbone, the high point, creating a lifting, V-shape effect that actually makes the lower face appear slimmer and more refined. Unwanted width only occurs when filler is placed on the sides of the cheek rather than the top.
What is the difference between midface filler and cheekbone filler?
Midface filler is placed in the lower cheek ("apple" area) to restore lost volume, support the under-eye, and soften nasolabial folds. Cheekbone (lateral/malar) filler is placed higher along the zygomatic bone to create sharp, defined, structural elevation. Many patients benefit from a combination of both, planned together for a natural overall result.
How soon can I sleep on my side after cheek filler?
We recommend sleeping on your back with your head slightly elevated for the first 2-3 nights after filler treatment. This prevents pressure from shifting the product while it integrates with surrounding tissue. After 72 hours, normal sleeping position can be resumed.
Can cheek filler help with my jowls?
Yes, indirectly. By restoring midface volume, cheek filler creates a structural "tenting" effect that gently lifts the skin of the lower face, subtly improving mild jowling and nasolabial folds as a secondary benefit. For more significant jowling, a combination approach addressing both the midface and lower face may be recommended during your consultation.
