Identity data: first name and surname
Contact data: telephone number
Transaction security data: the date and time the form was submitted

No health data is collected through the website form. Health data is processed only within the clinic, during the consultation process, and within the scope of the applicable legislation.

Purposes of processing

The data collected is processed in order to respond to your appointment request, to inform you about the appointment and to keep a record of the request.

If consent for commercial electronic messages has been given, that consent is assessed separately and applies only to promotional messages. Consent may be withdrawn at any time.

Legal basis

Responding to an appointment request relies on the legal basis set out in KVKK Article 5/2-c, namely that processing is necessary for the conclusion and performance of a contract. Sending commercial messages relies on explicit consent.

Transfers

Your personal data is not transferred abroad. Within Türkiye, data may be transferred only to authorised public institutions where required by law.

Retention period

Contact data relating to an appointment request is retained for the period prescribed by the applicable legislation, starting from the conclusion of the request. At the end of that period the data is deleted or anonymised.

Your rights as a data subject

Under KVKK Article 11 you have the right to learn whether your personal data is being processed, to request information if it has been processed, to request its correction or deletion, and to request that processing be restricted.
